The Opportunity *Full-time Hourly Contract Role We're looking for a driven Care Navigator who thrives at the intersection of empathy, communication, and execution. You'll support Medicare-age patients by helping them follow their care plan, overcome barriers (including social needs), and stay engaged with their care team and program. This is a Full-time remote position, Monday–Friday, 9am–5pm ET.
