WHAT IT IS We are looking for an RN to deliver patient centered health coaching and chronic disease management via chat, phone, and video. This role includes triaging symptoms, providing education on diabetes, hypertension, hyperlipidemia, and weight management, coordinating care with providers and cross functional teams, and documenting patient care accurately in a telehealth setting.
