We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity. While this role is remote, candidates must reside and licensed in Louisiana or Tennessee. T ravel is estimated to be up to 10% and expected schedule requirements are 4x10s 8:00 am - 6:00 pm (Monday-Friday) local time. What You’ll Do As an APP with Imagine Pediatrics, you influence and ensure the best clinical care for your patients. Our goal is to increase the number of safe days at home by ensuring kids and families get what they need when they need it—reducing unnecessary medical visits and hospitalizations . You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including registered nurses, navigators, dietitians, pharmacists, psychiatrists, social workers, and therapists.
